Creating a form

Forms are used to collect data from users. A survey-type form allows you to collect any information, while a user registration form enables users to sign up for classes themselves.

To create a form:

1. Expand the “LUDO” tab from the side menu.
2. Then select “Forms”.
3. Click the “New form” button.

4. In the form editor window, select the form typeSurvey — and give it a name. After completing these steps, the system will automatically generate a link to the published form.
NOTE:
To add questions or other elements to the form, simply drag selected components from the right-side panel into the main form area.
Each added element can be edited by changing its name, available options, and whether it is required. The range of available settings may vary depending on the type of element selected.
